it isn't until Kurtz' death that he recognizes what it is he has done. There were probably flashbacks of the "civilized" colony which he tried to form. He was probably seeing the heads of dead people on the poles he owned. These last words spoken by Kurtz tie up and end the story altogether. Because throughout this whole novel there is nothing but darkness in the forest, in the rivers, the secrets and people's hearts.
